ELIGIBILTY REQUIREMENTS * *

Eligibility Requirements: I hereby represent and warrant that I meet all of the eligibility requirements listed below: (a) I am a legal U.S. resident of the fifty (50) United States or District of Columbia, and am least eighteen (18) years of age. (b) Neither I nor any of my immediate family members (spouse, ex-spouse, parents, siblings, children, step-children, step-parents, step-siblings) or household members (whether or not related) currently are, or have been within the past one (1) year, an employee, director or officer of: i. The producer ("Producer") of the prank television show reflected in this application ("Program"), its licensees, assigns, parents, or affiliated or subsidiary companies, or any person or entity directly involved in the development, production, distribution or other exploitation of the Series or any variation thereof; ii. Any network or platform authorized to exhibit the Program (“Network”), any television station owned or operated by the Network, and each of their respective parents, subsidiaries, affiliates or related entities; iii. any known, major sponsor of the Series; or iv. any person or entity supplying services or prizes to the Series. (c) I am not currently a candidate for elected office, and if selected to participate in the Series, I agree that I will not become a candidate for elected office until at least one (1) year after the initial broadcast or other exhibition of the final episode of the Series in which I appear. (d) I have not been convicted of a felony in the past ten (10) years (unless expunged). (e) I do not currently (at or prior to signing this Agreement) have any confidential knowledge of or access to any material, assignments, challenges or competitions for the Series. I further understand and agree that Producer has the sole discretion to make determinations of eligibility in any and all respects, and that Producer reserves the right to change any of the eligibility requirements at any time. I understand and agree that Producer reserves the right at any time to render ineligible any person who Producer determines, in its sole discretion, is sufficiently connected with the production, administration or distribution of the Series such that their participation in the Series would create the appearance of impropriety. In addition, I acknowledge that Producer has informed me that all participants must be “up to date” in line with the CDC definitions of vaccination status, which is commonly accepted as having received a full round of vaccinations and a booster if eligible, with the last dose administered at least fourteen (14) days prior to participating in the show. Further eligibility requirements may apply and are subject to change without notice.

Application Video Release *

By signing this Release, I have voluntarily agreed to be filmed, recorded, or videotaped (including, without limitation, self-filmed, -recorded or -videotaped) in connection with my interest in being potentially considered to participate on the prank television show reflected in this application ("Program”). I hereby consent to the recording (including, without limitation, any self-recording), use and reuse by ITV America Inc., including its parents, subsidiaries, affiliates and assigns (collectively, “Producer”), any network or platform authorized to exhibit the Program (“Network”) and/or any of their respective licensees, assigns, parents, subsidiaries or affiliated entities and each of the respective employees, agents, officers and directors (collectively “Releasees”) of my voice (including, without limitation, my speaking and singing voices and musical compositions created by me), actions, likeness, name, appearance and biographical material, and any information contained in, derived from, or obtained in connection with my application to be a participant in the Program (collectively “Likeness”), as edited, altered, or modified by Producer, Network, or Releasees, in any and all media now known or hereafter devised, worldwide in perpetuity, in or in connection with: (i) my being considered for possible participation in the Program and (ii) the Program and associated promotion, marketing, advertisement, and/or other exploitation of the Program (in any form or media) thereof. I agree that Releasees may use all or any part of my Likeness and may alter or modify it regardless of whether or not I am recognizable. I specifically agree that Releasees shall have full exclusive ownership of any right, title, and interest in and to any and all recordings and video taken of me in connection with my application or the Program (the “Material”), and I shall have no rights whatsoever to the Material. I further agree that Releasees shall have the right to use, all or in part, the Material and my Likeness in any and all media now known or hereafter devised worldwide, in perpetuity. I grant the rights hereunder whether or not I am selected to participate in the Program in any manner whatsoever. I hereby irrevocably release Producer, Network, and their Releasees from any and all liability arising out of their use of my Likeness and/or the Material, my participation in the application and participant selection process, and/or any exploitation of the Program, my Likeness, or the Materials on any legal theory whatsoever (including, without limitation, personal injury, rights of privacy and publicity, false light and defamation) (the “Released Claims”). I agree not to make any claim against or sue Producer, Network, or their Releasees as a result of the recording or use of my Likeness and/or the Material or any claim based on any of the Released Claims. I understand that neither Producers nor Releasees make any representation that such Material will or will not be used in any way.
